1. 畢業證書：最高學歷之中文版或英文版

2. 工作證明：至少五年的工作經歷(碩博士畢業生就學期間實習可計入)

3. 英文個人資料表、學習研究計畫書：請下載範本 Word 或 Odt

4. 英文推薦函二封：格式不拘

5. 英文歷年成績單

6. 英文能力證明：TOEFL 80以上或GMAT 600以上。若無提供英文語言能力證明，則由英文面試。

7. 其他有助審查之資料 (英文文件如：特殊優秀事蹟等）

※報考資料及報名費：分成兩階段，請先完成UTA報名手續(與UTA台北辦公室聯繫)、再完成清大報名手續(12/8-12/18線上報名)。

[International In-Service Students]

1. One copy of your highest degree's diploma (in Chinese or in English) 

2. Working Certificate: You should have 5 years of working experience or above (internship experience can be counted in the period of studying for a master's or doctoral degree)

3. Personal Profile Form, Statement of Purpose: Please download the file Word or Odt

4. Two Recommendation Letters

5. One copy of your official transcript(s) (in English)

6. Language Certificates: You earned one or more of the following subsection scores: --above TOEFL iBT 80; --above GMAT 600. (Only international students need)

7. Verification of Nationality: verification of your nationality or a copy of your passport. (Only international students need)

8. Declaration Form (Only international students need): Please download the file Word

9. Financial Statement (Only international students need)

10. Certification of Financial Responsibility Form (Only international students need): Please download the file Word

11. Additional Supporting Documents (Optional): such as exceptional deeds.
